All athletes who plan to participate in CCA-CFA athletics MUST have a Sports Physical (form can be downloaded below) uploaded into FinalForms & completed the documents in FinalForms (instructions on how to create an account below:

• Current Physcial (English) / (Spanish) – to be completed by a physician
